How to make an ACC claim?
Step 1: Make an appointment with one of our physiotherapists
Step 2: Fill out the ACC45 claim form (available at an Axon clinic) with your Physiotherapist
Step 3: Your Physiotherapist sends the form to ACC for processing
Step 4: ACC will contact you about your claim to let you know if it has been accepted or declined
What do you have to pay?
There will be a surcharge for Physiotherapy or Acupuncture treatment under ACC. Please note that ACC will cover part of the treatment cost but does not cover all costs.
- Initial Physiotherapy treatment – $30
- Subsequent Physiotherapy treatment $15 per session
- Acupuncture Treatment ACC surcharge is $12 per session
- Private treatment (Physiotherapy and Acupuncture) $95- $75
Please note that a non-attendance charge ($30 ) fee will be made where an appointment is missed without any 24hour prior notice.
Do I need a Doctor’s referral?
No, we have all relevant ACC documentation so no doctor referral is required.
What conditions do you treat?
We treat a range of musculoskeletal and neurological conditions. For example: Muscle, joint and spinal problems, whiplash injuries, vertigo and headaches, sports injuries, arthritis and degenerative conditions, work related disorders, stroke, multiple sclerosis, traumatic brain injury, Parkinson’s disease, spinal cord injury, and facial palsy.

Are you registered with ACC or other insurance companies?
Yes, we are registered with ACC and most major insurers.
What hours do you work?
The clinic is open Monday to Friday between 8 am and 7 pm and Saturday between 9 am and 1pm.
If you require an appointments outside these times they can be arranged.
What makes you different?
We specialize in long-standing (chronic) problems that remain resistant to treatment and use a team of highly qualified specialized staff who use a movement-based approach to understand why you continue to have problems. Our evaluations are designed to identify the source of your problem, and give you the key to preventing re-injury.
Is there a cancellation fee?
If you fail to provide 24 hours cancellation notice or simply do not turn up to your appointment, Axon Physio & Acupuncture reserves the right to charge $30 to $50 for the missed appointment.
What forms of payment do you accept?
We accept payment by cash, EFTPOS or credit card. If you have Southencross health insurance, please bring your membership card with you.
